An Integrated Digital Platform for Personalized Care, Data Privacy, and Predictive Health Insights.

About the Project

The proposed project aims to develop an integrated digital platform based on advanced technologies such as DT (Digital Twin), Solid (Social Linked Data), and PODs (Personal Online Datastores). This Cloud-based platform is designed to improve the management and treatment of diabetes mellitus while providing patients with tools to manage their health data and to cope with legal demands coming from GDPR as well as national data protection legislation. By utilizing ML (Machine Learning) models and patients' personal data, the system will enable accurate simulations to determine the diabetes risk index and optimized therapeutic interventions in terms of dietotherapy or improved physical activity. Additionally, patients will be provided with dedicated, secure personal spaces for autonomous data management, ensuring full control and respect for their privacy by exploiting federated learning so that no data is exchanged but weights of the locally trained models. The main objectives include the evolution of the digital model by integrating various data sources, continuous and predictive monitoring of vital parameters, personalization of therapies, improvement of patients' quality of life, and promotion of patient autonomy and empowerment.
